I'm interested in hearing from others on how they manage Registration Codes.
Do people use a data base or a spreadsheet ?
or, do they use some specific software ?
If specific software, which one - and how do they find it to use ?
Any other advice on 'not getting into a mess' !

If you are just starting out, the easiest route to go would probably be with a spreadsheet. It's easy to use and you can perform simple searches.
If you expect your customer base to grow significantly, want to run complex queries, or if you want to be able to automate customer data modification and retrieval (i.e., such as for customer registration code requests), then you may want to go with some sort of robust database that can be programmatically queried by various means.
In any case, if you start with a spreadsheet, you can always export the data from the spreadsheet to a generic format that can be imported into a database.
Also, make sure you backup your data on a regular basis, whatever method you use.

I'm also interested in how other authors are handling this issue ...
particularly those who are using a software package designed for this purpose.
Look forward to some ideas.
